Your dedicated Care Designer, who is a Registered Nurse, works closely with an Occupational Therapist to ensure every modification fits your personal needs and goals. We guide you through the assessment process and help with the paperwork.

This scheme provides upfront, separate funding, meaning you can access essential safety equipment, from simple grab rails to smart safety devices, without using up your daily care budget.
Eligibility is determined by your standard My Aged Care assessment. If you are being assessed for Support at Home, you will automatically be evaluated for the AT-HM Scheme. If you already have a care plan but your needs have changed, we can help you request a re-assessment to access this additional support.
While the government provides an allocation to cover these costs, a small co-contribution may apply depending on your financial situation (e.g., self-funded retiree vs. pensioner). We will always provide a clear, transparent quote before any work begins so there are no surprises.

I do not have photos or videos to post, but I can say without fear of contradiction that Nurse Next Door , Joondalup, is an excellent care agency. I moved to WA from Queensland in around July this year and have found the agency to go above and beyond in helping me with the confusion of moving to WA which has many procedures different to Queensland. Without the help of staff I would have been completely lost in the maze of differences I have encountered, but staff knowing the difficulty of settling into a new state have been wonderful. I would also like to say, that as an elderly person with a terminal illness, their care has been outstanding. I could not have wished for better. There were some small issues in the early stages getting to know a new care agency and settling into a new state, but these were sorted with care and patience, for which I am very grateful. Saral and her team at Nurses Next Door are absolutely amazing. I have had other service providers before but nothing of this standard. Saral's and her team are such caring and empathetic people. I always feel very safe yet in control. The attention to my safety and best interest is obviously their priority. They are assisting me keep a positive routine on my NDIS path.
